TERMS AND CONDITIONS FOR PARTICIPATION
  1. The cost per person for participating in the 5th Nationwide Forum of Listed Companies’ Boards on 29 – 30 March 2012 totals: PLN 1,900 + VAT. An application for participation of three or more people from the same company or institution will give entitlement to a discount of 10%.
  2. The price contains attendance at the lectures, conference materials, coffee breaks, lunch and participation in the “Wrocław Evening” meeting. The cost of hotel accommodation is not included in the price.
  3. Upon receipt of the application we will send a confirmation of participation and a proforma invoice (if such option has been chosen) to the e-mail address indicated in the application.
  4. You are asked to make the payment within seven days as of the date of receipt of the confirmation of participation or the date of receipt of the proforma invoice. The payment should be made into the account indicated in the confirmation of participation or in the proforma invoice.
  5. Upon posting of the payment a VAT invoice will be issued and sent by mail to the indicated address and recipient.
  6. A withdrawal from participation should be sent to the organizer’s e-mail address – biuro@forumzarzadow.pl
  7. In the case of a withdrawal 14 days before the start of the Forum, we will charge the Participant with an administrative fee of PLN 500 + VAT. In the case of a withdrawal seven days before the Forum 100% fee for participation in the Forum will be charged.
  8. Failure to withdraw the application and non-attendance at the Forum will result in charging the full cost of participation.
  9. Failure to make the payment will not be regarded as commensurate with a withdrawal from participation in the Forum.
  10. It is possible for another employee of the institution to take part in the Forum instead of the original applicant, provided that such replacement’s personal data have been sent by e-mail or mail seven days before the start of the Forum.
  11. The organizer reserves the right to make changes in the programme.

We authorize Biuro Informacji Kapitałowych sp. z o.o. sp.k. to issue a VAT invoice without the recipient’s signature.

I hereby declare that the amount due will be transferred into the account of Biuro Informacji Kapitałowych sp. z o.o. sp.k.: Deutsche Bank PBC S.A. 13 1910 1048 2415 0308 1552 0001 within the time limit specified in the invoice. We also declare that we have got acquainted with the terms and conditions for participation in the forum.

Pursuant to the Personal Data Protection Act of 29 August 1997 (consolidated act, Dz. U. /Journal of Laws/ No. 101, item 926 of 2002) Biuro Informacji Kapitałowych sp. z o.o. sp.k. hereby informs you that it performs the role of a personal data administrator.

We consent to the processing of personal data for the purposes of promotion and marketing of the activity run by Biuro Informacji Kapitałowych sp. z o.o. sp.k.

The person granting consent to the processing of his/her personal data has the right to inspect and process the data concerning such person, including the right to correct these data.
